SOME (not *all*) cd writing softwares (actually .iso creating ones) will "sign" the created disc/.iso.

Start looking in sector 0x10. There is often plain text information that may answer your question.

Depends on what you mean. The pre-mastering software (roughly, the program that creates the .ISO or equivalent) sometimes breaks the rules, and puts its name and version in the System Identifier field of the Primary Volume Descriptor. (See ISO 9660 or the equivalent ECMA 119 for the technical details.)
Some software also has quirks that makes it possible to identify it just by looking at how files are laid out on the CD. Don't know any systematic study of this, though,
The software that actually puts the .ISO image onto the CD may not leave any traces behind – as far as I know noone has studied that.
