What do you guys use for extracting and looking at the hex of a SIM?
USIM Detective has the capability to record the APDU requests sent to the card and the data that is returned. All in hex too !
I see your company doing the training for it too!
Where is the usual location of the PIN and PUK code please on a SIM Card?
The SIM is a logical device, so when you are "reading" the card, you are sending requests to the SIM and it is replying.
It isn't a "hex dump" where you're seeing all the data from start to finish. You'll have to ask the SIM for its PIN. I have a feeling that it won't tell you!

Lastly, three other points
- Not every SIM Manufacturer reveals the EF containing the address for PIN validation; not every SIM reader reveals all the EFs in tree under the Master File on SIM / USIM.
- AlexC makes good observations relating to PIN. I do use a bit-nibbler also when needed.
- Lastly, if your question is leading to "is it possible during a read to reveal the PIN of a SIM or does the EF file header assist?" then of course you are asking for the answer to the SIM PIN Challenge connected with the first link above.
