Which UK law governs admissibility?

Ok, so I understand that you need to show continuity of evidence or else the evidence is not admissible in court, but does anyone know which UK law or act dictates this? PACE section 78 is all about unfairly obtained evidence, but does altered evidence come into this section too ?
