There are dozens of it on the internet, with its characteristic timestamp headline (time of printout) and the paging invariably in English, so it must be a common or default setting somewhere.

Maybe Guidance copied it from somewhere, but it's quite unlikely that the people who generated the PDFs online (or the source of my original printout) did this using EnCase. Fortunately, I don't need the solution to the riddle anymore. If someone knows the answer, it's still appreciated, of course.
